1. Fax Veterinarian Referral Form. Please fax the Veterinarian Referral Form (PDF) prior to having your client call us. Where possible, please include the patient’s most recent medical history and all copies of lab work done.
2. Call Ahead. Please call ahead. Instruct your client to call us for an appointment.
